The Potential Client Intake Sheet form is in Adobe.pdf format, allowing you to type in the information requested in the various fields. If you have Adobe PDF, you can download the Potential Client Intake/Information Sheet and save it to your computer. Then you can click on the right upper button “Highlight Fields” and type in the appropriate information.
The Client Intake Sheet is for our firm to use to determine what your needs are and whether it would be beneficial for you and the firm to move further and schedule an actual interview/consultation. The firm charges $200 for an actual interview/consultation with an attorney, which lasts approximately 1-2 hours.
You will not be charged for submitting a Potential Client Intake Sheet or for our review of the information. In addition, the mere submitting of this sheet does not create an “attorney-client” relationship between you and the firm. This firm cannot undertake representation until the proper agreements, understandings are made and documents signed. This generally will not happen until after the consultation/interview, if any.
The Information Sheet is not intended to intrude on your privacy and consists of the same questions you are asked when you call into our firm. This is simply to narrow your information down. If you are uncomfortable answering any field, just write” I wish not to answer” or a similar response.
All information is protected under privilege, and will remain confidential. Thompson & Bishop, in virtually all cases, represents wronged employees, and so has no known conflicts of interest with any employer, other than CVS Pharmacy and Steen Electric, Inc. (If you work for either of these companies, please find another firm to represent you and do not submit a sheet.)
